How to Use These Templates
- Open our free genogram maker
- Start with the basic symbols from the toolbar - Male, Female, or other types
- Follow the structure described in the template below
- Click each person to edit their name, dates, and notes in the properties panel
- Use the Relationship tool to connect partners, then add children from the relationship panel
- Click Auto Layout to organize everything neatly
- Export as PNG from the Menu when finished
Basic Three-Generation Template
The most common clinical genogram. Includes grandparents, parents, and the client generation. Start with two grandparent couples at the top, connect their children in the middle row with marriage lines, then add grandchildren at the bottom.
Generation 1: Paternal Grandparents - Maternal Grandparents
Generation 2: Father - Mother (married)
Generation 3: Client + Siblings (oldest left, youngest right)
Blended Family Template
For families with divorce and remarriage. Shows the original partnership (marked as divorced), the new partnership, and children from both relationships. Common in child welfare, custody evaluations, and family therapy contexts.
Father - First Marriage (divorced) - Mother
Father - Second Marriage - Stepmother
Children from first marriage + Children from second marriage
Student Assessment Template
A minimal template for social work or counseling students completing a family assessment assignment. Focuses on the client, their immediate family, and one generation above. Add notes for relevant health or behavioral information per your assignment requirements.
Generation 1: Parents (connected with marriage line)
Generation 2: Client (index person) + Siblings
Add: Names, ages, key notes for each person
Medical Genogram Template
Focused on hereditary health conditions. Include three or more generations with cause of death noted for deceased members. Use the Notes field to record diagnoses, chronic conditions, and mental health history. This template is commonly used in genetic counseling and primary care settings.
Generation 1: Grandparents (mark deceased with X, note cause of death)
Generation 2: Parents, Aunts, Uncles (note chronic conditions)
Generation 3: Client + Siblings + Cousins (note current health)
